Professional Referral
Adult Social Care in Sefton are offering you a new way to refer into our service. The following referrals are now available online:
- Safeguarding and Care Quality Concerns (non-urgent)
- Social Care
- Occupational Therapy
- Deprivation of Liberty Safeguards (DoLS) Forms 1 and 2
The new webpage will enable you to complete the details online and receive a notification when we have received your referral.
The form includes text fields and dropdown options to make the process as simple and user friendly as possible.
How to Use
To submit the information you will need to create an account. For individual professionals we recommend an individual login with your own email as the contact.
For further guidance on how to refer online, please read our guide for professionals.
For our providers we would encourage you to create this account using the business name and a generic email address, this will enable them pick up referrals should colleagues need to hand over work.
This development is part of Adult Social Care’s plan to offer a wider range of channels for you to communicate with the service and manage the needs of service users and patients.
