Disabled children's Information & Database

Welcome to Sefton's Disabled Children's Information Database

What is the database for?

To provide families of disabled children with information on local and national services.

 

It is also an opportunity to collect information to help with the planning of local services.

 

How do I know if my child should be registered?

If your child has a learning, physical, sensory or communication difficulty, chronic illness or medical condition and consequent/associated emotional or behavioural difficulty.

 

The categories of disability on the data collection form are:

  • Developmental Delay,
  • Physical Disability,
  • Chronic Illness/Medical condition,
  • Learning Disability,
  • Hearing Impairment,
  • Visual Impairment,
  • Communication Difficulties.

 

 

Why should I put my child's details on the database?

  • You will receive an information pack, which will provide you with details of current services in Sefton, including statutory and voluntary agencies.
  • You will be kept informed about changes in services, new services and other helpful information via a newsletter produced by the Co-ordinator.
  • Agencies involved in the provision of services will gain a better understanding of what type of services are needed now and in the future.

 

How do I put my child's details on the database?

To register your child's details you need to contact the Database Co-ordinator by phone, email or complete the attached application form. Mail it to the co-ordinator (address on the right of this screen) who will then arrange a home visit to complete a data collection form.

 

How can I find out about activities and events for families of disabled children/young people?

When you register you automatically receive a copy of the newsletter delivered to your home.

 

The newsletter is produced 3 times a year. However information is received continually.  The notice board will keep you up to date about events etc not included in the newsletter.

 

Are there any other useful websites?
