Health & Social Care Directorate Inspection

Following the Care Quality Commission inspection in December 2009 the report has now been published and is available to read on the Care Quality Commission website.

Sefton was rated excellent for improving the quality of life for older people which has been achieved by involving residents and their carers in the development of services and working with health agencies to provide rehabilitation services.

The report also says that Sefton is doing well to provide a range of community based support, for people to remain safely in the community, and is also making services more personalised for people.

Thank you to all those who supported and contributed towards the inspection process.

Dignity in Care

Within Sefton we are encouraging all health and social care workers to take action in their place of work to promote dignity in care. Within Sefton we have, together with partners, undertaken a promotional campaign to encourage members of the public and people who provide health and social care to sign up to become Dignity in Care Champions. As part of the campaign we helped signpost people to organisations who can help should they, or someone they know, have a concern that they are not being treated with dignity and respect.

Prevention and Early Intervention Strategy 2009 - 2010

Within Health and Social Care we are making a strategic shift towards prevention and early intervention for Sefton citizens. A Prevention and Early Intervention Strategy was launched in October 2009.

The Strategy has been produced by Health & Social Care Directorate to represent a Sefton partnership approach for the provision of, and access to, preventative services.

The Strategy provides web links to a menu of different preventative services in the borough, which will offer information, and practical support services to help adults lead healthier independent lives within their own community.

The Prevention and Early Intervention Strategy is part of the national Transforming Adult Social Care agenda taking place across the UK.

What we do

Sefton Health and Social Care Directorate, together with Sefton Primary Care Trust and partner organisations are committed to improving the health and well being of its residents, ensuring that people get the right treatment, at the right place, preventing the need for further treatment or hospital admission, helping people to live a full and active life within their community.

In January 2006 the Government issued a White Paper 'Our Health, Our Care, Our Say; a new direction for care services' which has personalisation at it's heart. The intention was to layout a lasting vision to reform and improve health and social care services which are flexible, integrated and responsive to people's needs and wishes, promoting independence, dignity, respect, choice and control.

The White Paper clearly sets out the direction of travel for Health and Social Care services over the next five years based on the concept of well being and strongly recommends the notion of ‘seven outcomes’ for Health and Social Care services as:

  • Improved health and emotional well-being;
  • Improved quality of life;
  • Making a positive contribution;
  • Choice and control;
  • Freedom from discrimination;
  • Economic well being;
  • Personal dignity.

As part of the Council’s Corporate Performance Management Framework, the Health & Social Care Directorate is required to produce a service plan for the coming year. The Plan includes actions for the next three years and a set of actions for the coming year based on the Seven Outcomes for Social Care. If you have any comments about the service plan please complete and return the feedback sheet.

Other Key Documents

There are a number of other key documents, recently produced which will help in shaping the future of social care services.

In 2007 the Government produced a paper "Putting People First - a shared vision and commitment to adult social care". This sets out the principles underpinning the sector wide commitment to transforming services for individuals and those family members and friends who support them. Person Centred Planning and Self Directed Support are integral to the future of social care.

In June 2008 the Government produced a strategy for Carers entitled "Carers at the heart of 21st-century families and communities - A caring system on your side. A life of your own." This strategy sets out the framework for developing support for carers over the next 10 years. It recognises the increasingly important role that carers play in supporting family and friends. It also recognises that carers need more personalised support and greater scope to control and customise services. Over the next few years the Government have committed to giving every person using social care services, including carers, a personal budget that can be tailored to meet agreed outcomes for the carer and the person who is being cared for.

In 2008 the Department of Health produced guidance for local Authorities:  "Transforming Social Care" - LAC (DH) (2008) 1. The key focus is to make personalisation, including a strategic shift towards early intervention and prevention, the cornerstone of public services. In social care this means every person across the spectrum of need, having choice and control over the shape of his or her support, in the most appropriate setting. It recognises that some people will need a significant levels of assistance to exercise choice and control. For more information about Sefton's Transforming Social Care Agenda.

As part of its drive to improve health and wellbeing across England, the Government have asked all NHS Primary Care Trusts and councils to carry out a Joint Strategic Needs Assessment (JSNA). In Sefton this has been called "Better Life Better Health". Consultation with local people has looked at four priority areas identified: children and young people, long term conditions, older people and health inequalities. All comments and feedback will now help Sefton PCT, Sefton Council and their partners in the voluntary and community sector develop their long term plans to improving services for Sefton residents.

Choose well this winter

NHS Sefton is encouraging local people to ‘Choose well’ by choosing the right treatment for their needs this winter when they are unwell. The Sefton Choose well winter health campaign is launching for its second year. Choose well highlights the importance of selecting the appropriate NHS service if you fall ill during the winter period — the time when demand for NHS services is greatest. Information also includes what to do if your GP surgery is closed.
