Adult Social Care
Sefton Adult Social Care Department, together with Sefton
Primary Care Trust and partner organisations are committed to
improving the health and well being of its residents, ensuring that
people get the right treatment, at the right place, preventing the
need for further treatment or hospital admission, helping people to
live a full and active life within their community.
In January 2006 the Government issued a White Paper
'Our Health, Our Care, Our Say; a new direction for care
services' which has personalisation at it's heart. The
intention was to layout a lasting vision to reform and improve
health and social care services which are flexible, integrated and
responsive to people's needs and wishes, promoting independence,
dignity, respect, choice and control.
The White Paper clearly sets out the direction of travel for
Health and Social Care services over the next five years based on
the concept of well being and strongly recommends the notion of
‘seven outcomes’ for Health and Social Care services as:
- Improved health and emotional well-being;
- Improved quality of life;
- Making a positive contribution;
- Choice and control;
- Freedom from discrimination;
- Economic well being;
- Personal dignity.
As part of the Council’s Corporate Performance Management
Framework, the Adult & Social Care Directorate is
required to produce a
service plan for the coming
year. The Plan includes actions for the next three years and a set
of actions for the coming year based on the Seven Outcomes for

Other Key Documents
There are a number of other key documents, recently produced
which will help in shaping the future of social care
services.
In June 2008 the Government produced a strategy for Carers
entitled
"Carers at the heart of 21st-century families and communities - A
caring system on your side. A life of your own." This strategy
sets out the framework for developing support for carers over the
next 10 years. It recognises the increasingly important role that
carers play in supporting family and friends. It also recognises
that carers need more personalised support and greater scope to
control and customise services. Over the next few years the
Government have committed to giving every person using social care
services, including carers, a personal budget that can be tailored
to meet agreed outcomes for the carer and the person who is being
cared for.
In 2008 the Department of Health produced guidance for local
Authorities:
"Transforming
Social Care" - LAC (DH) (2008) 1. The key focus is to make
personalisation, including a strategic shift towards early
intervention and prevention, the cornerstone of public services. In
social care this means every person across the spectrum of need,
having choice and control over the shape of his or her support, in
the most appropriate setting. It recognises that some people will
need a significant levels of assistance to exercise choice and

As part of its drive to improve health and wellbeing across
England, the Government have asked all NHS Primary Care Trusts and
councils to carry out a Joint Strategic Needs Assessment (JSNA). In
Sefton this has been called "
Better
Life Better Health". Consultation with local people
has looked at four priority areas identified: children and
young people, long term conditions, older people and health
inequalities. All comments and feedback will now help Sefton PCT,
Sefton Council and their partners in the voluntary and community
sector develop their long term plans to improving services for
Sefton residents.
